Consumer Request Form

As a California, Colorado, Connecticut, or Virginia resident, you have the right to submit a privacy request to know what personal information has been collected, used, and disclosed; to request deletion of your personal information; and to request the correction of inaccurate personal information we are maintaining about you. Please complete this form to submit your request.

To process your consumer request, we need some basic information from you. We will only use personal information provided to verify your identity or authority to make the request, to locate the information within our systems and respond to your request. We cannot respond to your request or provide you with personal information if we cannot verify your identity or authority to make the request and confirm that the personal information relates to you.

Please be aware that if you do not provide documentation, we may require you to provide documentation after your request if our own records indicate that the personal information you seek to correct is accurate.
